With the new CMS Requirements of Participation, “discharge” has become a major focus of the new survey process. Facilities face more scrutiny in regard to “facility-initiated transfer or discharge.” There are six (6) specific reasons that a facility can use for resident discharge, all needing explicit documentation by the interdisciplinary team.
